Professional Liability/Malpractice Insurance and Claims History
Effective July 1, 2016 – all University of Arizona residents and fellows transitioned to Banner Employees. Their Professional Liability Insurance is now covered by Banner Health for the duration of their residency/fellowship commencing after July 1, 2016.
Banner Malpractice Claims History
- Claim verification requests after July 1, 2016
- Complete the Banner Request for Claims History and Malpractice Insurance form and mail it to the address on the form.
University of Arizona Malpractice Claims History
- Claim verifications for University of Arizona residents and fellows before July 1, 2016
- Email the specific request and attach the University of Arizona Authorization to Release Form and send to gme@medadmin.arizona.edu
- Liability Insurance Information (Letter from UA Risk Management outlining coverage before July 1, 2016)
