Use this form to request new computing hardware, software or accessories. You may download and fill the form out online, then print it out and bring it to the Service Desk, or email it to servicedesk@medicine.arizona.edu.

Template to request funding for a new commitment and recruitment.

This form is to be filled out to request any official name changes, date of birth and social security corrections, or offical gender change. This form must be returned to Kris Slaney, Registrar, Room 2112 in order to ensure this change is reflected across all University systems.

Although rarely granted, students who wish to change their clerkship rotation site after assignments have been made may only apply for a change of site if they have an extreme hardship. Requests will be considered on a case-by-case basis. Students must complete a Change of Clinical Site form and submit it to clerkship program coordinator

This form is to authorize the release of information and records about your academic career at the College of Medicine - Tucson.

Affiliates (also called Designated Campus Colleagues) are select individuals from outside agencies, institutions and hospitals that have been afforded contractual "affiliate” status by the University's Office of the Provost.

Affiliate Verification Form (COM)

This form is required for students enrolling in independent study (899) and research (800) electives. It applies primarily to fourth-year MD students, and to some third-year students as well.
