Professional Liability/Malpractice Insurance and Claims History
Effective July 1, 2016, all University of Arizona residents and fellows transitioned to Banner employees. Their professional liability insurance is now covered by Banner Health for the duration of their residency/fellowship commencing after July 1, 2016.
Banner Verifications
For claim verification requests after July 1, 2016, please complete Banner Request for Claims History and Malpractice Insurance form and mail to the address on the form.
University of Arizona Verifications
For claim verifications for University of Arizona residents and fellows prior to July 1, 2016, please email specific requests and attach the University of Arizona Authorization to Release form to gme@medadmin.arizona.edu.
